Health history forms at Park School

As a new school year begins, staff across all seven Kids Alive schools in the DR have been meeting one-on-one with parents gathering health history forms for the 1,900 students in our care.

These meetings allow an opportunity for parents to share about their child’s physical health, emotional well-being, and the realities of their home environment. For some families, that means talking about asthma or recurring illness; for others, it may be struggles with behavior, changes to family at home, learning challenges, or concerns about nutrition.

By listening carefully to parents before the first day of classes, our multidisciplinary teams—teachers, counselors, health staff, and social workers—gain a clearer understanding of each child’s background and needs. This information will become a vital tool for providing the kind of wraparound support that defines our holistic model.

These conversations also strengthen the partnership between families and schools. Parents leave knowing their voices matter, and that their child is seen not only as a student, but as a whole person—loved, valued, and supported.

The forms provide incredible opportunity for gospel hope. In one conversation, a staff member at ANIJA School shared the gospel with a mother during a meeting. The mother, in turn, decided to give her life to Christ.

For Kids Alive DR, completing health history forms is more than an administrative step. It is an act of care—one that reflects our commitment to serve with excellence and to walk with children and families toward healing, growth, and hope in Christ.
